How do I get a substitute social security card?

You can receive a free social security card if for some reason you are without yours. You want to do so, you will have to fill in the application. You will need to provide additional documents showing your age, identity and citizenship. You will then need to send these items to the appropriate device.

The first step in obtaining a substitute social security card is to fill in the application. You can get an application from the Social Security Office. The form can also be downloaded from the Social Security website. The file that contains the application is called SS-5.

The application contains instructions and provides more information you need. Although five -page long, only one page must be completed and sent. The application will ask basic questions you should know or be able to get answers. This includes your name, address and social security number.
