The clinical significance of gastric mucosal dysplasia is:
(1) Mild dysplasia: Mild dysplasia is the hyperproliferative response of the mucosa to the injury, which often appears at the edge of ulcers, or various types of gastritis, proliferative polyps, and alopecia gastritis, etc. Most of the types are regenerative . Most of these diseases are reversible and do not require regular follow-up.
(2) Moderate dysplasia: The histological and cytological dysplasia of moderate dysplasia is relatively obvious, and it can appear in atrophic gastritis, adenomatous polyps, etc., as well as in the adjacent mucosa of cancer. Although some cases are reversible or remain intact for a long time, some can be aggravated and upgraded, so regular gastroscopy is needed.
(3) Severe dysplasia: Severe dysplasia has obvious histological and cytological atypia, which is sometimes difficult to distinguish from highly differentiated cancers in the mucosa. This lesion is mainly seen in adenoma-like polyps, cancerous mucosa, and occasionally in the tumor itself. There is a clear tendency to malignancy, and there is very little chance to recover from degeneration. Therefore, recent gastroscopy biopsy and close follow-up observation should be performed.
