What Is Hydralazine?
Hydrazine is used in patients with renal hypertension and higher diastolic blood pressure. The effect alone is not very good, and it is easy to cause side effects, so it is often combined with reserpine, hydrochlorothiazide, guanethidine or propranolol to increase the efficacy.

Hydralazine indication
- 1. Hypertension: As long-term use can cause serious adverse reactions, currently this product is rarely used in the treatment of hypertension. To offset the reflex tachycardia and sodium water retention caused by it, -blockers and Diuretics. 2. Congestive heart failure: V-HeFTI test shows that patients with heart function grade to congestive heart failure are taking diuretics and digitalis plus 75 mg of hydralazine 4 times a day and 40 mg of isosorbide daily. 4 times, in addition to improving symptoms, and reducing mortality (but failed to reach statistical significance).
Hydrazine pharmacological action
- This product directly relaxes small arterial smooth muscle. Its role mainly comes from reducing afterload. It can increase the content of cGMP in vascular smooth muscle cells by activating guanylate cyclase (cGMP), so that smooth muscles relax, small arteries dilate and reduce peripheral Vascular resistance and dilated veins are small. The drug has a prominent role in expanding coronary arteries, kidneys, brain and visceral arteries. It has a direct positive inotropic effect on the myocardium and an excitatory -receptor effect directly or because of histamine release.
- medicine interactions
- Diazoxide or other antihypertensive drugs can enhance the antihypertensive effect of this drug. 2. Sympathomimetic amines can reduce the antihypertensive effect of this medicine. 3. Non-steroidal anti-inflammatory analgesics can reduce the antihypertensive effect of this drug. 4. Food can increase the bioavailability of this medicine, so it should be taken after meals. 5. Combined with propranolol (propranolol) can increase the maximum plasma concentration of propranolol by two times. When combined, the dose of propranolol (propranolol) should be reduced. 6. Combined with furosemide can shorten the half-life of furosemide, because the drug increases renal blood flow; 7. Combined with indomethacin (indomethacin) can reduce the hypotensive effect of hydralazine.
- Pharmacokinetics
- Gastrointestinal absorption is irregular, about 50% of the protein binding. Almost all are metabolized in the liver. The half-life is about 10-12 hours. The effect starts within 30 minutes after oral administration, and the effect lasts about 4-8 hours. Mainly excreted by the kidneys, <2% are excreted in the urine in the original form, and 2% are excreted in the feces.
- Dosage form and specifications
- 1. Tablets: 10mg, 25mg, 50mg, 100mg; 2. Hydrazine dosage usage
- Oral or intravenous injection, intramuscular injection: generally start with a small amount, 10mg each time, 4 times a day, medication for 2 to 4 days, the dosage gradually increased in the future. In the first week, 25 mg each time, 4 times a day, after the second week, 50 mg each time, 4 times a day (if it exceeds 400 mg per day, it is easy to produce adverse reactions) [2] .
Prediction of hydralazine
- 1. Drug resistance and adverse reactions such as headache, palpitations and nausea may occur after taking the medicine. 2. This product can cause rheumatoid arthritis and lupus erythematosus-like reactions when used in large doses for a long time. 3. Disabled in patients with coronary artery disease, cerebrovascular sclerosis, tachycardia and angina pectoris. 4. Adverse reactions include orthostatic hypotension, fever, dizziness, sweating, and dyspnea.
Contradiction of hydralazine
- Patients with lupus erythematosus, connective tissue disease, and severe angina pectoris are contraindicated. Patients without coronary heart disease with atherosclerotic heart disease, angina pectoris, allergic to hydralazine, and patients with aortic dissection.
