Pharmacological effects of mupirocin

This medicine is a topical antibiotic for external use. It is a substance produced by the culture medium of Pseudomonas fluorescens, namely Pseudomonas A. Its antibacterial effect is mainly through the reversible combination with isoleucine transfer RNA synthetase to prevent the infiltration of isoleucine and stop the synthesis of isoleucine-containing protein in cells to play a bactericidal or bacteriostatic effect. The drug shows bacteriostatic effects at very low concentrations and bactericidal effects at high concentrations. This medicine is highly sensitive to various gram-positive cocci related to skin infections, especially staphylococci and streptococci, and it is also effective against drug-resistant staphylococcus aureus. Neisseria gonorrhoeae has a certain antibacterial effect; it is not sensitive to most anaerobic bacteria and normal skin flora. The incidence of resistant strains in vitro is very low.

Pharmacokinetics of mupirocin

After being applied to the skin, the drug can penetrate into the stratum corneum, and the binding rate with human serum protein is 95%. After absorption, it can be rapidly metabolized into inactive first acid (monic acid A) and excreted by the kidneys. When administered orally or by injection, it is rapidly metabolized to inactive metabolites after absorption and loses its effect.

Mupirocin indication

It is suitable for skin infections caused by various bacteria, especially Gram-positive cocci, such as secondary bacterial infections based on pustular disease, rickets, folliculitis and eczema, dermatitis, various types of ulcers and trauma.

Mupirocin contraindications

Those who are allergic to this medicine and other ingredients of this medicine are contraindicated.

Mupirocin precautions

1. Use with caution: use with caution in patients with moderate or severe renal impairment, pregnant women. 2. This medicine ointment is not suitable for intraocular or intranasal use. If it gets into the eye, it should be rinsed with water. 3. This medicine excipient polyethylene glycol has potential nephrotoxicity, which may increase absorption when used in large-scale burns, nutritional ulcers, etc. At this time, special attention must be paid. 4. This drug has no cross-resistance with other antibiotics, including methicillin-resistant strains, which are almost all sensitive to this drug. 5. May cause slight elevation of aminotransferase.

Mupirocin adverse reactions

Local application is generally no adverse reactions, only occasional skin burning, stinging, itching, etc., generally mild, without stopping the drug.

Mupirocin Dosage

Topical application: apply to the affected area, dressing or covering with dressing if necessary, 2 to 3 times a day, 5 days as a course of treatment. Repeat 1 course if necessary.

Mupirocin efficacy review

Mupirocin ointment is highly effective in external treatment of bacterial skin diseases, with small adverse reactions, a clinical success rate of 96%, and a bacterial clearance rate of 94%, all exceeding the efficacy of internal use of diclocillin or erythromycin [3] .
