Surgical treatment
As long as the metastatic adenocarcinoma meets the indications for surgery, surgical treatment should be preferred. This plays an important role in prognosis and prolonged life, improves quality of life, and reduces re-metastasis. Postoperative chemotherapy or radiotherapy is used to improve the treatment effect. Indications for surgical treatment of metastatic adenocarcinoma: one or several lung metastases are confined to one piece; the primary tumor has been completely controlled; there are no metastatic tumors in the body other than the lesion and no other effective treatment method; the patient is generally in good condition and has no surgical contraindication certificate. The surgical method of metastatic adenocarcinoma is based on the principle of maximal preservation of the original organs. Local or wedge resection should be performed as much as possible. When recurrence of adenocarcinoma metastasis occurs, re-excision of the organ is reserved.
2. Radiation therapy
All patients with metastatic adenocarcinoma who are not suitable for surgery and have certain sensitivity to radiotherapy are feasible. Radiation therapy also suppresses cancerous pain and can be used in patients with trachea, esophageal compression or pain. Radiation-sensitive tumors include: seminoma, malignant lymphoma, nephroblastoma, neuroblastoma, Ewing sarcoma, etc. Lung metastases from breast and head and neck cancers are also sensitive to radiation, but radiotherapy is not the first choice.
3. Chemotherapy
Chemotherapy for metastatic lung cancer is an indispensable treatment method, especially for primary tumors that are sensitive to chemotherapy, such as villous epithelial cancer, small cell lung cancer, etc., even without surgical resection, it has a considerable effect. For patients with disseminated metastatic lung cancer or with pleural effusion, chemotherapy should be selected. The treatment plan should choose chemotherapy drugs that are more sensitive to the primary tumor for combined chemotherapy. Some chemotherapy drugs have pulmonary toxicity and should be avoided.
4. Interventional Therapy
Through puncture technology, chemotherapy drugs are directly injected into the blood vessels that provide blood supply for metastatic cancer, including the dual supply of pulmonary and bronchial arteries, and the abundant blood supply is the pathological basis of interventional therapy. Generally, the pulmonary femoral arterial infusion chemotherapy is first performed through the left femoral vein. After the end, the puncture site is used to compress the hemostasis. Indications for interventional therapy: The primary tumor has been well controlled, there is no metastasis of other parts of the body, it is not suitable for surgical treatment, and there are no contraindications to intervention. Those with extensive distribution of both lungs should be treated with bronchial artery infusion chemotherapy on both sides, and those with one or one lobe distributed should be treated with corresponding bronchial artery infusion chemotherapy. The dual interventional treatment of metastatic tumors is a safe and effective treatment method. Infusion of chemotherapy drugs will not cause damage to the heart and normal lung tissue.
