What Is a Uterine Perforation?
1. Sudden abdominal pain during uterine cavity surgery.
Uterine perforation
- Uterine perforation refers to the damage of the entire uterine wall caused by uterine cavity surgery, causing the uterine cavity to communicate with the abdominal cavity or other organs. Uterine perforation is most common in female genital tract injuries. Can be seen in placing or removing IUDs, abortions,
- 1. Sudden abdominal pain during uterine cavity surgery.
- 2. It was found that the depth of the instrument used to enter the uterine cavity was significantly greater than the depth of the uterine cavity estimated at the time of examination, without resistance, and the resistance of the uterine wall was not felt.
- 3. Lower abdominal tenderness and rebound pain.
- 4. If the large blood vessels are damaged by perforation, the typical manifestations of internal bleeding can occur in a short time and occur quickly.
- Diagnose based on
- 1. The patient suddenly felt pain in the lower abdomen, abdominal tenderness and rebound pain during the operation.
- 2. During the operation, the surgeon suddenly felt that the resistance of the uterine wall was lost, and there was no sense of bottom. The depth of the instrument entering the uterine cavity far exceeded the actual depth of the uterine cavity.
- 3. If you see fat tissue or bowel oozing out, the diagnosis is no doubt.
Uterine perforation treatment principles
- 1. Conservative treatment: Close observation in hospital. If the tissue of the uterine cavity has been scraped and there are no signs of internal bleeding, uterine contractions and antibiotics can be given. If the uterine cavity tissue has not been aspirated, the perforation is small, there is no obvious internal bleeding, and the patient is in good condition, you can ask an experienced doctor to avoid the perforation and scrape the tissue before conservative treatment, or to cure the infection after a week.
- 2. Surgical treatment: If there are obvious signs of internal bleeding or suspected organ damage, surgery should be performed immediately (laparotomy).
Uterine perforation medication principles
- 1. Small perforation (such as probe perforation) and no other organ damage to oral metronidazole, motherwort extract or injection of oxytocin.
- 2. Large perforations should not be treated with drugs alone, especially when combined with other organ damage or internal bleeding.
