Rosenberg's team first reported tumor infiltrating lymphocytes (TIL) in 1986. TIL cell phenotype is heterogeneous. In general, most cells in TIL are CD3 positive. Among tumor-derived TIL cells, the proportion of CD4 + T cells and CD8 + T cells is different. In most cases, CD8 + T cells are predominant. The percentage of CD25 + cells in freshly isolated TIL was low. With the increase of IL-2 culture time in vitro, the percentage of CD25 + cells gradually increased. The NK cell markers (CD16, CD56) tend to increase first and then decrease during TIL plus IL-2 culture.
The tumor infiltrating lymphocytes were isolated from the tumor by mechanical treatment and enzymatic digestion, and cultured with high dose IL-2 in vitro. All the remaining tumor cells died from 7 to 13 days. Compared with LAK cells derived from PBMC, IL-2 activated TIL is characterized by: (1) 50-100 times, so the amount of effector cells and IL-2 can be reduced in the treatment, and advanced tumors still have a certain therapeutic effect ; (2) mainly induced by CD8-positive cells, and found that TIL has a specific tumor-killing effect in animal experiments; (3) the host's inhibitory state is conducive to the killing effect of TIL, so cyclophosphamide (Cy ) 100mg / kg can improve the curative effect, which may be related to the elimination of suppressive cells or factors by immunosuppressive drugs, and enhanced the effect of adoptive immunotherapy, so it can reduce the amount of IL-2 and reduce toxic and side effects; Lymphocytes were obtained from tumor tissue, tumor draining lymph nodes, and cancerous pleural and ascites fluids. After cultured with IL-2, their growth and expansion ability was stronger than that of LAK cells. TIL has been reported to treat 14 patients with advanced tumors such as metastatic lung cancer, of which 4 tumors have shrunk by more than 50%, and the side effects are lower than IL-2 / LAK therapy.
