What Is a Vagotomy?

Gastric vagotomy has been widely used in the treatment of simple ulcers abroad. According to Dragstedt's summary of 800 cases of vagalectomy, this method is considered to be an effective and safe surgical method, which can replace subtotal gastrectomy for duodenal ulcer. But in general, there is no conclusion about the definite signs and long-term effects of vagus nerve amputation.

Gastric vagotomy

There are three types of gastric vagotomy:
Vagus nerve amputation
Is the earliest operation. At the level of the esophageal hiatus, the left and right ventral vagus nerve trunks are separated clearly, and
Advantages of highly selective gastric vagotomy:
Eliminates neurological gastric acid secretion. Eliminates the main cause of recurrence of ulcer disease.
Preserving the tension and peristalsis of gastric antrum without additional drainage.
The function of pyloric sphincter is retained, and the chance of bile reflux and dumping syndrome is reduced.
The normal volume of the stomach is retained without affecting the food intake. The operation is simpler and safer than major gastrectomy, and the operation is not more complicated than selective vagustotomy.
This is a newly developed operation with increasing applications. In theory, it should be the ideal surgical method for treating duodenal ulcers, but there are still some problems, such as anatomical variations of the vagus nerve, cutting is not easy to ensure safety, and nerve regeneration has not been solved, so there are still some Recurrence rate. In addition, a wide range of blood vessels can be used to separate the small curvature of the stomach, which may cause ischemia, necrosis, and perforation of the small curvature of the stomach. The treatment of gastric ulcer is not as good as that of major gastric resection. The clinical application is still short, and the actual effect needs to be further observed.
